Following your consultation, our staff will be pleased to assist you with planning your treatment program and providing you with the information you need to ensure the best results for you.  Our procedures also require we obtain a complete an accurate medical profile and other related information before any treatment program can begin.

Please take some time and fully complete the registration and other forms we require before you return  for your first treatment. Please bring these along with you at the time of your first visit and please do not hesitate to call or email us if we can assist you with any questions.  The necessary forms are currently available from our technicians or treatment consultants at the time of your initial consultation.  In the future you will be able to register online with us and forms can be downloaded and printed at home.
